Rhode Island DEM oil spill, UST & LUST file review

Choose Emergency Response, UST or LUST custody before requesting a DEM file. A site inventory is a lead, not delivery of the reports.

Before you search

Start with the right identifiers.

Street address, municipality, historic facility name, incident/facility/site reference, dates and the exact documents needed.

Bring what you have. Missing identifiers can be part of the route review.
What you are looking for
The specified program's existing incident/cleanup reports, not just an inventory line.
If the result is empty or incomplete
Ask the coordinator which program was searched. Keep an Emergency Response oil-spill file separate from UST/LUST registration and cleanup files. Adapt the request outline ↓
01

Use inventories to identify the file

DEM's site-inventory page separates record programs and published lists. Check which list and vintage supplied a candidate reference. A regulated UST or cleanup inventory is not a complete home-tank history, and a nearby site should not be assigned to the subject property without comparing identifiers.

Select the correct file-review program

DEM's file-review coordinator lists Emergency Response oil spills and aboveground registrations separately from UST and LUST files. Describe the incident and record family in the request. Asking for all tank paperwork without naming the program can leave the actual oil-spill response file unresolved.

Request inspection or copies of the missing records

Follow DEM's current file-review instructions and linked request form for the identified program. State whether you need inspection or copies and name the report and date range. Keep scheduling, acknowledgment, fees and documents delivered as separate events; no appointment or message is sent from this page.

After the search

What should you actually come away with?

The specified program's existing incident/cleanup reports, not just an inventory line.

Program and reference

Compare town, location, event dates and the DEM program named in the reply.

Still missing? A UST-only search may not answer the Emergency Response question.

File delivery

List the report and attachments actually inspected or received.

Still missing? A scheduled review or acknowledgment is not document delivery.

Before you interpret the result

Which DEM program should an oil-spill request name?

DEM lists Emergency Response for oil-spill responses, alongside separate UST and LUST files. Use the event and known reference to identify the appropriate program.

Your private record worksheet

Keep the next step clear.

Prepare search keys here, open the official sources above, then record what happened. Nothing is searched or sent automatically. A postal city or ZIP does not establish legal jurisdiction.

Suggested document scope for this route: the specified DEM Emergency Response, UST or LUST reports and related final correspondence. Narrow this to the files you still need.

What happened at each source?

These are your notes, not verified agency findings. Leave a source as “Not checked” until you use it.

Your next actions

Start with the identifiers and first applicable source. An online blank, an inaccessible portal and an agency no-record reply are three different outcomes.

No result is not proof of no tank. A candidate document still needs its property, issuer, dates and stated scope checked.

Make the request specific

A ready-to-adapt outline.

Please provide access to existing [Emergency Response oil-spill / UST / LUST] records for [address; town], [reference], [dates], specifically [reports and correspondence]. Please identify the correct program if these records are held elsewhere.

Replace the bracketed fields and follow the agency’s current form, disclosure, and fee instructions. This is a draft, not a sent request.

Keep the limits visible.

Program inventories and file-review coordination do not prove a file exists for every residential tank. Agency access requirements and charges apply independently. Self-service only.
